I hereby acknowledge this release from liability for accidental injury or illness which I may incur as a result of participating in any physical activity. I hereby assume all risks connected therewith and consent to participate in this program. I agree to disclose my physical limitations, disabilities, ailments, or impairments which may affect my ability to participate in this program. I acknowledge that this activity may involve a test of a person's physical and mental limits and may carry with it the potentiaI for death, serious injury, or property loss.

I understand that classes are nonrefundable and any chargebacks will result in a lawsuit. I agree to idemnify and hold harmless against any and all claims, suits, actions of any kind for liability, damages, compensation or otherwise brought by me or anyone on my behalf, including attorney's fees and any related costs, if litigation arises pursuant to any claims made by me or anyone acting on my behalf. If I incur any of these types of expenses, I agree to reimburse.

I understand and consent to the filming of classes that may or may not be posted in the form of pictures, videos, or voice recordings taken by Wade Bryant or other authorized assistants, participants, volunteers, or spectators.
